Paranoid schizophrenia is the most common type of schizophrenia in most parts of the world. The clinical picture is dominated by relatively stable
(i.e persistent and unvarying), often paranoid delusions, usually accompanied by hallucinations, particularly of the auditory variety, and perceptual disturbances. Disturbances of affect, volition, and speech, and catatonic symptoms, are not prominent. The paranoid schizophrenic may appear otherwise fully functional, but is plagued with impossible beliefs and wildly imagined fears. Examples of the most common paranoid symptoms are:
1. delusions of persecution - such as beliving that Nazi psychologists are using radio controlled bacteria to attack one's kidneys
2. delusions of reference, i.e. they believe unrelated events and discussions pertain to them personally, and secret discussions and documents relating to them are being held in secret by nefarious agencies whose sole purpose is to persecute them.
3. delusions of exalted birth,, such as believing oneself to be a deity, orrspring of a deity or a an exalted person, such as LRH, or that one is the grandaughter of President Eisenhower, or one was born in a "secret underwater base" called Chattanooga in the Great Salt Lake.
4. delusions of special mission, such as saving the earth from alien invader forces, evil galactic emperors, or other imagined ills and woes, such as "thetans" or "engrams" or a global conspiracy of psychologists and psychiatrists. 5. delsuions of bodily changes, such as believing one has been kidnapped and one's memory was wiped or altered, or that one's kidneys are being destroyed by remote controlled bacteria, or that radio impants have been placed in one's brain.
6. delusions of loss or jealousy, such as believing one's spouse was kidnapped and murdered by government agents to persecute you and all evidence of their existence removed and hidden away in places like university libraries and government agfencies to which you send requests for files on yourself that don't exist.
7. hallucinatory voices that threaten the patient or give commands, or auditory hallucinations without verbal form, such as whistling, humming, or laughing;
8. hallucinations of smell or taste, or of sexual or other bodily sensations; visual hallucinations may occur but are rarely predominant.
9. Thought disorder may be obvious in acute states, but if so it does not prevent the typical delusions or hallulcinations from being described clearly.
10. Affect is usually less blunted than in other varieties of schizophrenia, but a minor degree of incongruity is common, as are mood disturbances such as irritability, sudden anger, fearfulness, and suspicion.
11."Negative" symptoms such as blunting of affect and impaired volition are often present but do not dominate the clinical picture.
12. Delusions can be of almost any kind of delusions of control, influence, or passivity, but persecutory beliefs of various kinds are the most characteristic.
13. The course of paranoid schizophrenia may be episodic, with partial or complete remissions, or chronic.
14.In chronic cases, the florid symptoms persist over years and it is difficult to distinguish discrete episodes.
15.The onset tends to be later in life than in the hebephrenic form of schizophrenia, where the delusions lack coherent structure, or the catatonic form, where psychomotor distrubances are the predominate symptom.
